Chronic Disease Management

Every day at Construct Health we treat a number of people experiencing chronic illness or disease.

The list of chronic illnesses and their management is extensive but as an example can include:

  • education, gentle mobilisation and exercise planning for patients with long-standing chronic pain
  • breathing exercises and chest physiotherapy for cystic fibrosis and asthma patients
  • mobilisation and strengthening exercises for those with chronic back and neck pain
  • Pilates and progressive rehabilitation programs for those with osteoarthritis, osteoporosis and post-joint replacement surgery.

For many of our patients the financial burden of managing these conditions is significant. This is why the Australian government has created the Chronic Disease Management Program. This program is coordinated by the patient’s general practitioner (GP) and enables the patient to access five (5) physiotherapy or other allied health services per year.

Each patient is cared for under a GP Management Plan (GPMP) and through Team Care Arrangements (TCAs), an approach which ensures a more holistic, consultative and informed outcome for all involved.
